We at Janini IVF, the best IVF Centre in Delhi, sometimes practice a segmented approach, or like how many people call it, “Freeze-all” approach to IVF. Instead of transferring the embryos in a fresh cycle (same cycle wherein your eggs were retrieved) we freeze ALL your embryos. In other words we defer the transfer of embryos till about one month later.
The reasons are many. Indeed, many of the IVF clinics in America and other developed countries already employ this approach. They do not transfer any of the embryos in the fresh cycle at all, no matter how many good embryos have developed. The reason behind doing this is, when we transfer the embryos in a fresh cycle, the implantation rate is a little lower than a frozen cycle. The reason is not the embryos, it's the endometrium. As you may know that the embryos when transferred inside the uterus have to ‘interact’ with the maternal endometrium and attach to it to gain nourishment. It is vitally important for the endometrium to be “Receptive” so that it is in a state that can accept the incoming embryo(s). What happens in a fresh cycle is when we give gonadotropin hormones there is a slight disruption to this receptivity of the endometrium such that it becomes a little unable to do this function.
Imagine someone asks you to bake a cake. Now instead of using 200 degrees at 30 minutes if someone puts the oven at 500 degrees for 10 minutes you would know its not the same. The cake would burn. Something similar happens to the endometrium after receiving gonadotropin injections. It “shifts” in its time-frame to accept the embryo in a fresh cycle so if we transfer the embryo it is possible that even if the embryo was of a good quality, it would not implant. So rather than doing that we give a period of rest to the endometrium and use the frozen embryos to transfer them when the time is right. When the endometrium is more receptive. This approach has given us excellent results over a period of years and this is what we excel doing.
Not at all. Since the decision to go for a “Freeze-all” is ours we offer the first frozen-thaw cycle completely free of cost. So financially speaking, there won’t be any change in the cost whether you go for a fresh transfer or frozen transfer in the first attempt. It is like we are taking the complete burden of costs incurred for freezing your embryos on our shoulders as we don’t charge you for the same. Then why do we do it? Simple - to maximise your chances of success. Being the best surrogacy centre in Delhi NCR, we want every couple coming to us taking home a baby as soon as possible. Even if it means we have to spend from our pocket or take more effort for it. You will be charged only for the remaining embryos that stay after the first transfer is over. In other words, you would not pay extra for the first set of embryos that will be transferred, but only for subsequent transfers if the first transfer fails. And since we at Janini IVF have world-class results of almost 70% (range from 60-80% depending on a wide variety of factors) even if you fail the first transfer attempt, fret not. No need to worry - we will usually have more good quality embryos of yours frozen with us. Trust me, if you avail two or three transfer attempts with good quality embryos it is very unlikely that you won’t get a result. Most of our patients take a baby home and we are committed for it.
Again, freezing embryos have many advantages. Having extra embryos frozen would mean that you can simply go for another attempt of transfer without going for the whole IVF procedure. No need for those daily injections. No need for another procedure under anaesthesia. No need so spend money on another cycle because the cost of a frozen-thaw transfer is very nominal. It is like you get a whole new cycle (or 3 or 4 cycles) at the cost of one. Scientific evidence is replete that freezing and thawing embryos do not lead to any damage whatsoever. Our frozen embryo recovery is more than 99%.
So we have frozen embryos with us. You already got your menses and now we call you to start on some oral tablets that will prepare your endometrium lining. Once after 2 or 3 scans when we find your endometrium to be satisfactory (7-14 mm) we would then tell you a date when your transfer would happen. We have sophisticated methods of using Doppler parameters on the ultrasound to make double sure that your endometrium is optimal for transfer. On the day of the embryo transfer, we thaw your embryos, culture them for a few hours and do the transfer as usual. Other steps of properly taking the prescribed medicines and doing the test after 15 days to confirm pregnancy are the same as mentioned in IVF section
