Conventionally, an HSG or Hystero-salpingo-graphy test was ordered to check for the tubal patency. While still the gold standard of diagnosis, HSG test has its own pitfalls such as it is a painful technique and involves the instillation of a radio-opaque dye into the uterus, which is later on captured on an X ray. So, HSG requires an X ray test. HyCoSy or Hystero-salpingo contrast sonography, on the other hand, does not require the X ray at all and saves the patient from the radiations of an X ray.
HyCosy is basically a sonographic procedure. First a small saline is instilled inside the uterus by placing a small tube through the cervix. A trans-vaginal probe is inserted and thorough assessment of the uterus is made first. Any intra-uterine lesions like fibroids or polyps are ruled out. Then, a special sulphur based liquid, which is not radio-opaque but sono-opaque, is instilled which makes its way into the tubes. So, in this way, the fallopian tubes are clearly visualised by Sonography which would not have been possible otherwise (normally the tubes are not visible on sonography unless they are diseased). This helps in diagnosing any blocks if present in one or both tubes.
